(1)Application for an operator's license shall be submitted on a form furnished by the
department, and shall be filed no later than thirty
(30)days in advance of the next
examination date.
(2)The department shall notify the applicant within ten
(10)days of the examination
date whether or not the applicant has passed the examination.
