§ 81.55. How is the petition formatted and signed?

(a)Each page of the petition must contain:
(1)A summary of the purpose of the petition, or proposed document, or proposed amendment language;
(2)Numbered lines for each individual to print their legal name, current mailing address, date, and signature, and;
(3)The following declaration at the bottom of each page to confirm the collector was present when each signature was collected: "I, (Collector's Printed Name) , hereby declare that each individual whose name appears above signed and dated the petition. To the best of my knowledge, the individual signing the petition is a member of the tribe and is 18 years or older. (Signature of Collector) (Notary Certification)",
(b)Each individual must print their legal name, current mailing address, date, and sign on a numbered line.
(c)Each collector must complete and sign the declaration on each page in front of a notary, who will sign and certify.
