This is to certify that\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_ (Name of public utility concurring) assents to and concurs in the rate schedule (rate schedule supplement) described below, which the\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_ (Name of public utility filing rate schedule) has filed, and hereby files this certificate of concurrence in lieu of the filing of the rate schedule (rate schedule supplement) specified.
