(FDA)is correcting a notice that appeared in the **Federal Register** of October 14, 2014. The document announced that a proposed collection of information had been submitted to the Office of Management and Budget

(OMB)for review and clearance under the Paperwork Reduction Act of 1995. In this document, we correct some errors that appeared in the notice.
