SUPPLEMENTARY INFORMATION: The notice of a major disaster declaration for the State of Ohio is hereby amended to include Public Assistance for the following areas among those areas determined to have been adversely affected by the catastrophe declared a major disaster by the President in his declaration of June 3, 2004: Columbiana, Cuyahoga, Hocking, Medina, Noble, Perry, Portage and Summit Counties for Public Assistance (already designated for Individual Assistance.) Carrol, Guernsey, Harrison, Jefferson, and Knox Counties for Public Assistance. (The following Catalog of Federal Domestic Assistance Numbers
