432.093 Availability of voluntary acknowledgment of paternity form; responsibility of health care facility and parents.
(1)Any health care facility as defined in ORS 442.015 shall make available to the parent who gave birth to a child and the alleged genetic parent of any child born live or expected to be born in the health care facility, a voluntary acknowledgment of parentage form when the facility has reason to believe that the parent who gave birth to the child is unmarried.
(2)The responsibility of the health care facility is limited to providing the form and submitting the form with the report of live birth to the State Registrar of the Center for Health Statistics. The parent who gave birth to the child and the child’s alleged genetic parent are responsible for ensuring that the form is accurately completed.
(3)This form shall be as prescribed by ORS 432.098. [Formerly 432.285; 2025 c.592 §21]

432.093.
(1)Any health care facility as defined in ORS 442.015 shall make available to the parent who gave birth to a child and the alleged genetic parent, presumed parent or intended parent of any child born live or expected to be born in the health care facility, a voluntary acknowledgment of parentage form when the facility has reason to believe that:
(a)The parent who gave birth to the child is unmarried;
(b)The parent who gave birth to the child is married but the alleged genetic parent of the child is not the spouse of the parent who gave birth to the child; or
(c)The child was conceived by assisted reproduction, other than under a surrogacy agreement.
(2)The responsibility of the health care facility is limited to providing the form and submitting the form with the report of live birth to the State Registrar of the Center for Health Statistics. The parent who gave birth to the child and the child’s alleged genetic parent, presumed parent or intended parent are responsible for ensuring that the form is accurately completed.
(3)This form shall be as prescribed by ORS 432.098.
