An individual subject to an action taken under section 4734.31 of the Revised Code, other than permanent revocation of a license to practice chiropractic or certificate to practice acupuncture, may apply to the state chiropractic board to have the individual's license or certificate restored to good standing. The board may impose terms and conditions on restoration of the license or certificate by doing any of the following:
(A)Requiring the applicant to obtain training, which may include requiring the applicant to pass an examination upon completion of the training;
(B)Requiring the applicant to pass an oral or written examination, or both, to determine fitness to resume practice;
(C)Restricting or limiting the extent, scope, or type of practice of the applicant.
