774.26b Blank receipt forms; use; copies.
Sec. 26b.
(1)The county treasurer shall provide to each municipal court blank serially numbered receipt forms in triplicate, to be used if the court receives any money on account of a cash bail, bond, fine, penalty, or taxation of costs. The receipt forms shall provide space for recording the following information:
(a)The name of the defendant and payor.
(b)The name of the municipal judge.
(c)The docket number.
(d)The date.
(e)The amount of a fine received.
(f)The amount of costs received.
(g)Amount and nature of any other sum received.
(h)The total amount received.
(2)One copy of the receipt form shall be for the payor, 1 for the court, and 1 for the county treasurer.
