§ 12-6B-03

§12–6B–03.
(a)An applicant for registration shall:
(1)Submit an application to the Board on the form that the Board requires;
(2)Unless otherwise qualified under § 12-6B-02(a) or
(d)of this subtitle, provide documentation of the completion of a pharmacy technician training program under § 12-6B-02(b)(5) of this subtitle;
(3)Unless otherwise qualified under § 12-6B-02(a) or
(d)of this subtitle, provide documentation of having successfully completed an examination approved by the Board;
(4)Submit to a request for a State criminal history records check; and
(5)Pay the application fees set by the Board.
(b)The application shall be signed by the applicant.
