311.683 Application for license of inactive status -- Reactivation.

(1)A person licensed under KRS 311.674 and 311.675 may apply for inactive status
upon submitting an application and paying an inactive-status fee.
(2)An inactive license may be reactivated upon application to the board. If a license
has been inactive for more than five
(5)consecutive years, the licensed practitioner
shall apply for a new license and shall meet all the requirements in existence for a
license under KRS 311.674 and 311.675. That application for licensure shall
require:
(a)Evidence of the license holder's payment of an inactive-status fee; and
(b)Payment of the initial licensure fee.
