The offender level of the system shall consist of at least the following information:
(1)Name of the offender, including other names by which he is known;
(2)Date of birth;
(3)Sex;
(4)Race;
(5)Ethnic origin;
(6)Residence address at time of arrest; and
(7)Personal identification number.
