An application for registration or renewal of registration shall be accompanied by a fee in the following amount:
(1)An initial application for registration fee determined by the department, not to
exceed three hundred dollars ($300);
(2)A renewal fee determined by the department, not to exceed three hundred dollars
($300);
(3)An application for registration fee based upon certification of registration or
licensure issued by another state determined by the department, not to exceed two
hundred fifty dollars ($250); or
(4)An application for renewal of registration based on a renewal of registration in
another state, not to exceed two hundred fifty dollars ($250).
