15.876 Standardized forms for entry into state case registry.

All forms, child support orders, wage withholding orders, or orders amending an existing child support order, entered in any case in Circuit, District, or Family Court that require entry into the state case registry pursuant to KRS 15.802(3) shall be entered on forms adopted by the Administrative Office of the Courts in coordination with the Office of the Attorney General. If the provisions of a child support order are contained in an order that is narrative in nature, the adopted forms shall be used in addition to the narrative order.
