(1)Except for those voters who have been certified as requiring assistance in voting on
a permanent or annual basis, any person voting by means of a mail-in absentee
ballot or in-person absentee ballot who receives assistance in voting shall be
required to complete the voter assistance form required by KRS 117.255.
(2)Any person who assists another person in voting by use of an mail-in absentee
ballot or in-person absentee ballot shall complete the voter assistance form required
by KRS 117.255.
(3)The detachable flap on all mail-in absentee ballot envelopes shall have printed upon
it the voter assistance form required by KRS 117.255, notice of the penalty for
failure to complete the form, and notice of the penalty under KRS 117.0865.
(4)The State Board of Elections shall promulgate by administrative regulations under
KRS Chapter 13A a voter assistance form which shall be in a form acceptable to the
Attorney General.
