Sec. 16. Applications for certification shall:
(a)Be on forms prescribed and furnished by the Agency;
(b)Contain statements showing applicant's education and experience;
(c)Contain not less than 3 references; and
(d)Be accompanied by the proper fee as provided in Section 22 of this Act.
