Sec. 14. A person shall be qualified for initial licensure as an optometrist if that person has applied in writing in form and substance satisfactory to the Department and who:
(1)(blank);
(2)has graduated, after January 1, 1994, from a program of optometry education approved
by the Department or has graduated, prior to January 1, 1994, and has met substantially equivalent criteria established by the Department;
(3)(blank); and
(4)has met all examination requirements including the passage of a nationally
recognized examination authorized by the Department. Each applicant shall be tested on theoretical knowledge and clinical practice skills.
