Sec. 30. License applications; renewal procedures. An application for an original license or renewal shall be made to the Department in writing or electronically on forms prescribed by the Department and shall be accompanied by the required fee, which shall not be refundable. Any such application shall require such information as in the judgment of the Department will enable the Board and Department to pass on the qualifications of the applicant for a license. If the application for renewal with the required fee is not received by the Department before the expiration date, the existing license shall lapse and become null and void.
Failure to renew before the expiration date is cause for a late payment penalty, discipline, or both.
