Sec. 511.103. Application. The applicant for a license shall file with the Director an application upon a form prescribed by the Director, which shall include or have attached the following:
(1)The names, addresses and official positions of the individuals who are responsible
for the conduct of the affairs of the administrator, including but not limited to all members of the board of directors, board of trustees, executive committee, or other governing board or committee, the principal officers in the case of a corporation or the partners in the case of a partnership; and
(2)A non-refundable filing fee of $200 which shall become the initial administrator
license fee should the Director issue an administrator license.
