105.12 Form of license.

1. A contracting, plumbing, mechanical, HVAC-refrigeration, sheet metal, or hydronic license shall be in the form of a certificate under the seal of the department, signed by the director of the department, and shall be issued in the name of the board. The license number shall be noted on the face of the license.
2. In addition to the certificate, the board shall provide each licensee with a wallet-sized licensing identification card.
