Applications for licensure shall be on forms prescribed by the commissioner. The licensure fee for an environmental health specialist shall be eighty dollars for initial licensure. Each license shall be renewed annually in accordance with the provisions of section 19a-88 . The fee for license renewal shall be forty dollars.
