(a)A transfer of enrollment from one participating health plan to another may be made by a subscriber at times and under conditions as may be prescribed by regulations of the board.
(b)The board shall provide for the transfer of coverage of any subscriber to another participating plan
(1)if a contract with any participating plan under which the subscriber receives coverage is canceled or not renewed and
(2)at least once a year upon request in a manner as determined by the board, and
(3)if a subscriber moves to an area that the current health plan does not serve.
