Sec. 734. Inclusion of comprehensive contraceptive counseling in health assessment forms

Not later than 90 days after the date of the enactment of this Act, the Secretary of Defense, in consultation with the Secretary of Health and Human Services, shall— revise the periodic health assessment form of the Department of Defense (Department of Defense Form 3024) to include the information specified in subsection (b); and revise the pre-deployment health assessment form of the Department (Department of Defense Form 2795)— to allow members of the Armed Forces to indicate they would like comprehensive contraceptive counseling; and to include the information specified in subsection (b).
The information specified in this subsection is the following: An explanation of patient-centered contraceptive counseling as recommended by the American College of Obstetricians and Gynecologists, including by incorporating any clinical guidance on contraceptive counseling set forth by the American College of Obstetricians and Gynecologists. A description of the full range of contraceptive methods, including any contraceptive drug, device, or biological product approved, cleared, authorized, or licensed by the Food and Drug Administration under section 505, 510(k), 513(f)(2), 515, or 564 of the Federal Food, Drug, and Cosmetic Act ( 21 U.S.C. 355 , 360(k), 360c(f)(2), 360e, 360bbb–3) or section 351 of the Public Health Service Act ( 42 U.S.C. 262 )).
Such other information relating to contraceptive counseling as the Secretary of Defense determines appropriate.
