An identification of the nature of the allegation made in each such report, such as— fraud, waste, and abuse; harassment or other personnel issues; questionable intelligence activities; or threats to health and safety. For each such report— whether an investigation was initiated because of the report; for any such investigation, whether the status of the investigation is initiated, in progress, or complete; and for any completed investigation, whether the allegation made in the report was found to be substantiated or unsubstantiated, and whether any recommendations or criminal referrals were made as a result.
