The Center for Medicaid and Medicare Innovation
(CMMI)will establish a dedicated fund to identify, pilot, evaluate and scale delivery and payment models that target health disparities among racial and ethnic minorities, including models that support high-value non-medical services that address socially-determined barriers to health including: English proficiency, health literacy, case management, transportation and enrollment assistance which will help to reduce disparities and impact the overall cost of care. Pilot and demonstration programs that include partnerships with entities including community based organizations or other non-profit entities to help address socially determined barriers to health and health care will be given priority. Require all CMMI funded alternative delivery and payment models to include measures to assess and track their impact on health disparities, using existing measures such as, but not limited to, the National Quality Forum Healthcare Disparities and Cultural Competency Measures, and stratified by race, ethnicity, English proficiency, gender identity, sexual orientation, and disability status.
