Not later than July 1, 2015, the Secretary, in consultation with the Director of the Centers for Disease Control and Prevention, shall develop an online advance care planning toolkit. The advance care planning toolkit, which shall be available in English, Spanish, and any other languages that the Secretary deems appropriate, shall be maintained and publicized by the Secretary on an ongoing basis and made available on the websites of the following agencies: The Centers for Disease Control and Prevention.
The Department of Health and Human Service's National Clearinghouse for Long-Term Care Information. The Administration for Children and Families. The advance care planning toolkit shall include content addressing— common issues and questions regarding advance care planning, including individuals and resources to contact for further inquiries; advance directives and their uses, including living wills and durable powers of attorney; the roles and responsibilities of a health care proxy;
Federal and State-specific resources to assist individuals and their families with advance care planning, including— the advance care planning toll-free telephone hotline established under section 101; the advance care planning clearinghouses established under section 102; the advance care planning toolkit established under this section; available State legal service organizations to assist individuals with advance care planning, including those organizations that receive funding pursuant to the Older Americans Act of 1965 ( 42 U.S.C. 3001 et seq. ); and website links or addresses for State-specific advance directive forms; and any additional information, as determined by the Secretary.
