Section 704 (29 U.S.C. 796c) is amended— in subsection (a)— in paragraph (1)— by inserting after State plan the following: developed and signed in accordance with paragraph (2), ; and by striking Commissioner each place it appears and inserting ILA Director ; in paragraph (2)— in the matter preceding subparagraph (A), by striking developed and signed by ; and by striking subparagraphs
(A)and
(B)and inserting the following: developed by the chairperson of the Statewide Independent Living Council, the director of the designated State entity described in subsection (c), and the directors of the centers for independent living in the State, after receiving public input from individuals with disabilities throughout the State; and signed by— the chairperson of the Statewide Independent Living Council, acting on behalf of and at the direction of the Council; the director of the designated State entity described in subsection (c); and not less than 51 percent of the directors of the centers for independent living in the State. ; in paragraph (3)— in subparagraph (A), by striking State independent living services and inserting independent living services in the State ; in subparagraph (B), by striking and at the end; and by striking subparagraph
(C)and inserting the following: working relationships and collaboration between— centers for independent living; and entities carrying out programs that provide independent living services, including those serving older individuals; other community-based organizations that provide or coordinate the provision of housing, transportation, employment, information and referral assistance, services, and supports for individuals with significant disabilities; and entities carrying out the vocational rehabilitation program established under title I, and other programs providing services for individuals with disabilities; and cooperative agreements and partnerships to provide a seamless model for provision of services to individuals with disabilities and to avoid duplication of services. ; in paragraph (4), by striking Commissioner each place it appears and inserting ILA Director ; and by adding at the end the following: The State plan shall provide for the provision of independent living services on a statewide basis, to the greatest extent possible, including through the establishment of additional centers for independent living or focused outreach to serve underserved populations. ; in subsection (b), by striking the period and inserting the following: , as well as a plan for funding the administrative costs of the Council. ; in subsection (c)— in the subsection heading, by striking and inserting unit ; entity in the matter preceding paragraph (1), by striking the designated State unit of such State and inserting a State entity of such State (referred to in this title as the ; designated State entity ), which may be the designated State unit, an entity within the designated State agency, or an entity within a different State agency, in paragraphs

(2)and inserting the following: the Statewide Independent Living Council; centers for independent living; the designated State entity; and other State agencies or entities represented on the Council, other councils that address the needs and issues of specific disability populations, and other public and private entities determined to be appropriate by the Council. ; in subsection (m)— in paragraph (4), by striking Commissioner each place it appears and inserting ILA Director ; and in paragraph (5), by striking Commissioner each place it appears and inserting ILA Director ; and by adding at the end the following: The plan shall describe how the State will provide independent living services that promote full access to community life for individuals with significant disabilities. The services shall include— facilitating transitions of individuals with significant disabilities from nursing homes and other institutions, to home- and community-based residences, with the requisite supports and services; providing assistance to individuals with significant disabilities that are at risk of entering institutions so that the individuals may remain in the community; and facilitating transitions of youth (including students) who are individuals with significant disabilities, who were eligible for individualized education programs under section 614(d) of the Individuals with Disabilities Education Act (20 U.S.C. 1414(d)), and who have completed their secondary education or otherwise left school, to postsecondary life, including employment. .
