1. An amended certificate of marriage issued pursuant to NRS 122.137 must be substantially in the following form:

This is to acknowledge that a marriage officiant, did on the ........ day of the month of ........ of the year ........, at ................ (address or church), ................ (city), State of ................, date of birth ........, with their mutual consent, in the presence of at least one witness.
