Application for the writ shall be made, by complaint, either by the party for whose relief it is intended or by some person in his behalf, and shall be duly verified. The complaint shall allege
(a)the facts concerning the commitment, confinement or restraint of the person for whose relief the action has been instituted;
(b)the place of such confinement or restraint;
(c)the name of the person who has custody over him or by whom he is confined or restrained;
(d)the cause or pretense of such confinement or restraint, if known; and
(e)if the confinement or restraint is alleged to be illegal, in what the alleged illegality consists.
