The Administrator of the Federal Emergency Management Agency shall develop and establish a universal application for Federal disaster assistance for individuals in areas impacted by emergencies or disasters. In carrying out this section, the Administrator shall consult with the following: The Director of the Office of Management and Budget. The Administrator of the Small Business Administration. The Secretary of Housing and Urban Development. The Secretary of Agriculture. The entities described in paragraph
(1)shall provide prompt support to the Administrator. The application established under subsection
(a)shall include a voluntary survey to collect the demographic data of an applicant.
