This Act may be cited as the . Creating High-Quality Results and Outcomes Necessary to Improve Chronic (CHRONIC) Care Act of 2017 The table of contents of this Act is as follows: Sec. 1. Short title; table of contents. TITLE I—Receiving High Quality Care in the Home Sec. 101. Extending the Independence at Home Demonstration Program. Sec. 102. Expanding access to home dialysis therapy. TITLE II—Advancing Team-Based Care Sec. 201. Providing continued access to Medicare Advantage special needs plans for vulnerable populations.
TITLE III—Expanding Innovation and Technology Sec. 301. Adapting benefits to meet the needs of chronically ill Medicare Advantage enrollees. Sec. 302. Expanding supplemental benefits to meet the needs of chronically ill Medicare Advantage enrollees. Sec. 303. Increasing convenience for Medicare Advantage enrollees through telehealth. Sec. 304.
