Section 1705A is amended— in subsection (a)(1), by striking Any individual and all that follows through covered. and inserting the following: Any individual who applies for or seeks hospital care or medical services under this chapter shall, at the time of such application, or otherwise when requested by the Secretary, furnish the Secretary with such current information as the Secretary may require to identify any health-plan contract, as defined in subsection (i)(l) of section 1729, under which such individual is covered, to include, as applicable, the name, address, and telephone number of such health-plan contract; the name of the policy holder, if coverage under a health-plan contract is in the name of a person other than such individual; the plan identification number; and the group code of the plans. ; and in subsection (c)— by striking The Secretary and inserting
(1)Except as provided in paragraph (2), the Secretary ; and by adding at the end the following new paragraph: The Secretary may charge an individual who does not provide the information required by subsection
(a)reasonable charges for the provision of such care and services. .
